You must notify the local authority of any changes to the notification information within one month of the change, and if the device ceases to be a notifiable device you must notify the local authority in writing as soon as possible.

An application form can be downloaded from the document section of this page.
The application is for the purpose of placing information in a register kept by the Council. It would, however, be prudent to check if either Planning consent or a Building Warrant are required for the installation. The information kept in the register is primarily used by inspectors from the relevant enforcing authority to investigate outbreaks (two or more confirmed, related cases) thought likely to have arisen from wet cooling towers or evaporative condensers.
As this is simply a registration process, no permission is required.
